SMS Terms and Consent
How Oral Plastic Surgery obtains consent, what messages we send, and how to opt out.
Program Overview
Oral Plastic Surgery sends transactional SMS messages to patients who have provided express written consent. Messages include appointment reminders, appointment confirmations, recall notices, and post-operative follow-up communications related to the patient's care.
How We Obtain Consent
We collect SMS consent through three channels. In each channel, the consent option is presented unchecked by default, and the patient must affirmatively select it.
1. Patient Intake Form
At a patient's first appointment, they complete the practice's intake form (paper or digital), which includes a dedicated SMS consent section with an unchecked checkbox the patient must affirmatively select. The disclosure reads:
"I agree to receive SMS messages from Oral Plastic Surgery related to my care, including appointment reminders, confirmations, recall notices, and post-operative follow-up. Message frequency varies. Msg and data rates may apply. Reply HELP for help, STOP to opt out. Consent is not a condition of treatment."
2. Website Appointment Request Form
The appointment request form at oralplasticsurgery.com/appointment contains an unchecked SMS consent checkbox above the submit button with the same disclosure language. Submitting the form without checking the box still allows the patient to request an appointment; only the SMS consent itself is withheld.
3. Website Contact Form
The contact form at oralplasticsurgery.com/contact contains the same unchecked SMS consent checkbox above the submit button, with identical disclosure language.
Full Disclosure Text
Message types: appointment reminders, appointment confirmations, recall notices, post-operative follow-up.
Message frequency: varies. Typically 1 to 4 messages per month per patient.
Carrier charges: Message and data rates may apply.

Consent is not a condition of treatment. Declining SMS consent does not affect your ability to receive care.
What We Do Not Do
- We do not sell, share, or transfer phone numbers or SMS consent records to any third party for marketing purposes.
- We do not send promotional, marketing, or solicitation messages.
- We do not contact patients via SMS without prior written consent.
Privacy
